Manually create a bootable SD card for Raspian

I would like to know the steps used to make a SD card bootable for Raspian without using the ImageWriter program.
My first attempt was to use GPARTED and set the flags on a Fat16 partition to match the flags on a known bootable SD card, but that didn't work.

My intention, and I've already got this working on two Banana Pi's, is to boot to the operating system loaded on the hard disk drives.  The SD card is used solely to start the system.  I've even deleted the ext4 partition on the SD cards, and it works.  I've also changed the swap file location to the hard disk drive, so the SD does nothing more than start the system.
But creating the bootable SD using ImageWriter is a long and cumbersome process, and since all I need are the three files on the boot partition and the correct flag setting, I'd like to learn how to do it manually.
